63.2-501. Application for assistance.

Except as provided for in the state plan for medical assistance servicespursuant to 32.1-325, application for public assistance shall be made tothe local board and filed with the local director of the county or city inwhich the applicant resides. The application shall be in writing on formsprescribed by the Commissioner and shall be signed by the applicant underpenalty of perjury in accordance with 63.2-502.

If the condition of the applicant for public assistance precludes his signingan application, the application may be made on his behalf by his guardian orconservator. If no guardian or conservator has been appointed for theapplicant, the application may be made by any competent adult person havingsufficient knowledge of the applicant's circumstances to provide thenecessary information, until such time as a guardian or conservator isappointed by a court.
