46.2-490.10. Changes in form of ownership or name.
Any change in the form of ownership or the addition or deletion of a partnershall require a new application and license. The addition or deletion of aclinic site or change in the name of a clinic shall require immediatenotification to the Department and the Department may endorse the change onthe license as appropriate. The change of an officer or director of acorporation shall be made at the time of license renewal.
