3.1-249.39. Statement to be filed by registrant.

The registrant shall file with the Commissioner, a statement including:

1. The name and address of the registrant and the name and address of theperson whose name will appear on the label, if other than the registrant;

2. The name of the pesticide;

3. A complete copy of the labeling accompanying the pesticide and a statementof all claims made and to be made for it including directions for use;

4. If requested, a full description of the tests made and the results thereofupon which the claims are based; and

5. Such other information which may include, but is not limited to, productefficacy, all known health and environmental impacts, and known incidents ofhuman or wildlife illnesses, as determined by the Board.

In the case of renewal of registration, a statement shall be required onlywith respect to information which is different from that furnished when thepesticide was registered or last reregistered or is in response to additionalrequirements imposed by the Board.
