(1) (2) Beginning on January 1, 2001, upon issuance, delivery, or renewal of a health benefit plan in Kentucky, an insurer shall: (a) Clearly indicate on each covered person's identification card the mailing address where a claim for payment shall be sent; and (b) Issue new identification cards or an appropriate sticker to covered persons no later than thirty (30) calendar days following the effective date of any change in the address of the insurer, its agent, designee, or other entity that processes claims for the insurer. Identification cards for covered persons shall identify whether the covered person has health maintenance organization (HMO), point of service (POS), preferred provider organization (PPO), or indemnity fee for service (FFS) coverage.
