All insurers issuing health benefit plans in this Commonwealth that provide coverage for surgical services for a mastectomy and that are delivered, issued for delivery, amended, or renewed on or after July 14, 2000, shall also provide coverage for mammograms under KRS 304.17-316. The coverage shall meet the standards set forth in KRS 304.17-316. Effective: July 14, 2000 History: Created 2000 Ky. Acts ch. 18, sec. 2, effective July 14, 2000.
