(1) All landscape architects desiring to continue practice shall annually, prior to July 1, secure from the board a renewal certificate upon the payment of a fee and <br>completion of approved continuing education contact hours to be established by <br>administrative regulation of the board. The failure of any licensee to renew his <br>license annually in June shall not deprive him of the right of renewal, but the fee to <br>be paid for the renewal of a license after June shall be increased ten percent (10%) <br>for each month or fraction of a month that payment of renewal is delayed. The <br>maximum fee for delayed renewal shall not exceed twice the normal renewal fee for <br>each year the license is not renewed. Not later than May 1 of each year the board <br>shall notify by mail all landscape architects of the renewal date and fee. Application <br>for a renewal shall be upon a form to be established by administrative regulation of <br>the board. Each landscape architect shall furnish the information required by the <br>board. (2) Notice in writing shall be given to the board by each landscape architect of any change of a residence address within thirty (30) days. (3) Failure of any landscape architect to secure his renewal certificate within sixty (60) days after July 1 shall constitute sufficient cause for the board to revoke his license. (4) The board may restore a license that has been suspended for failure to pay the renewal fee or for failure to complete the continuing education requirements, upon <br>the receipt of payment of all applicable fees and receipt of verification of <br>completion of the continuing education requirements as provided in this section.
