2005 Arizona Revised Statutes - Revised Statutes §36-1906 Registering place of business with director
A. A person who holds a license shall notify the director in writing of the address of the place or places where he engages in the practice of fitting and dispensing hearing aids, and any change of address.
B. The director shall keep a record of the places of practice of persons who hold licenses. Any notice required to be given by the director to a person who holds a license may be given by mailing it to him at the address given by him to the director.
