304.17A-257 Coverage under health examinations and laboratory tests. (1) (2) benefit plan for colorectal cancer A health benefit plan issued or renewed on or after January 1, 2009, shall provide coverage for all colorectal cancer examinations and laboratory tests specified in current American Cancer Society guidelines for colorectal cancer screening of asymptomatic individuals as follows: (a) Coverage or benefits shall be provided for all colorectal screening examinations and tests that are administered at a frequency identified in the most recent version of the American Cancer Society guidelines for colorectal cancer screening; and (b) The covered individual shall be: 1. Fifty (50) years of age or older; or 2. Less than fifty (50) years of age and at high risk for colorectal cancer according to current colorectal cancer screening guidelines of the American Cancer Society. Coverage under this section shall not be subject to a separate deductible or separate coinsurance but may be subject to the same deductible or coinsurance established for other laboratory testing under the health benefit plan.
