OF YOUTH SERVICES Defendant {¶ 1} On complaint : : : : : : : : : : : : : : : : : : June against 15, 2005, defendant, plaintiff, Chris Department of Stone, Youth filed a Services. Plaintiff did not submit the filing fee or a poverty statement with the complaint. On June 16, and again on July 28, 2005, notices were sent to the plaintiff requesting that he submit the filing fee or a poverty statement. Plaintiff did not comply with either notice. On August 11, 2005, defendant filed the investigation report. On August 12, 2005, the investigation report was sent to plaintiff via certified mail. On August 19, 2005, the certified mailing was returned marked Youth no longer @ facility. {¶ 2} The form complaint clearly states: If you move or change telephone numbers you must give the Court written notice of the new address or telephone number. Plaintiff has failed to provide the court with his new address or to submit the filing fee or a poverty statement. Accordingly, plaintiff s case is DISMISSED without prejudice. The court shall absorb the court costs of this case.
