§ 5003 Materials for issuing agents

§ 5003. Forms
The Commissioner shall procure and send to each town and county clerk such forms and reports of uniform size, and with margin for binding, as are necessary to be used in compliance with the provisions of this part.

§ 5003. Materials for issuing agents
The State Registrar shall procure and send to issuing agents materials as may be necessary for the issuance of vital event certificates. (Amended 2017, No. 46, § 6, eff. July 1, 2019.)
