§ 1157. Diagnosis and treatment plan
(a) Within thirty days of the date of admission to the Weeks School, the commissioner shall prepare for each child a diagnostic and treatment plan which describes:
(1) The reasons for admission to the school;
(2) The family, community and school environment;
(3) An assessment of the medical, psychological, social, educational and, if appropriate, vocational needs of the child;
(4) The immediate and long-term goals of treatment;
(5) The chosen methods of treatment; and
(6) The expected duration of treatment.
(b) The diagnostic and treatment plan shall be reviewed and revised at least every thirty days during the time the child remains at the school.
(c) At the time of discharge the plan shall be rewritten to include a current assessment of the environment and needs of the child, an evaluation of progress made toward immediate and long-term goals and a description of the placement. (Added 1973, No. 232 (Adj. Sess.), § 5.)
