§ 4515a. Form and rate filing; filing fees

Every contract or certificate form, or amendment thereof, including the rates charged therefor by the corporation shall be filed with the commissioner for his or her approval prior to issuance or use. In addition, each such filing shall be accompanied by payment to the commissioner of a nonrefundable fee of $50.00. (Added 1985, No. 236 (Adj. Sess.), § 10; amended 1991, No. 166 (Adj. Sess.), § 10.)
