2006 Utah Code - 75-2-1110 — Notification to physician.

     75-2-1110.   Notification to physician.
     It is the responsibility of the declarant or other signer of a directive to notify or provide for notification to attending physicians of the existence of a directive made under this part. Attending physicians who are notified shall make the directive or a copy of it a part of the declarant's medical records.
