§ 4730.28. Application for restoration of certificate.
 

(A)  An individual whose certificate to practice as a physician assistant has been suspended or is in an inactive state for any cause for more than two years may apply to the state medical board to have the certificate restored. 

(B) (1)  Before restoring a certificate under this section, the board shall determine the applicant's present fitness to resume practice. The board shall consider the moral background and the activities of the applicant during the period of suspension or inactivity. 

(2) When restoring a certificate, the board may impose terms and conditions, including the following: 

(a) Requiring the applicant to obtain additional training and pass an examination upon completion of the training; 

(b) Restricting or limiting the extent, scope, or type of practice as a physician assistant that the individual may resume.
