1. A person licensed pursuant to this chapter shall obtain a duplicate of that license if the:
(a) Original was destroyed, misplaced or mutilated; or
(b) Name or address of the licensee has changed.
2. To obtain a duplicate license a person must:
(a) File an affidavit with the Board, on the form prescribed by the Board, which states that the original license was destroyed, misplaced or mutilated or that the person s name or address has changed; and
(b) Pay a fee of $25.
