Applications, form, contents, fees.

336.040. Every person who desires to obtain a license to practice optometry shall apply to the board, in writing, on forms prepared and furnished by the board. The application shall state the required qualifications of the applicant, that the representations are made under oath or affirmation and are true and correct to the applicant's best knowledge, subject to the penalties of making a false affidavit or declaration and shall be accompanied by the examination fee.
