Section 11-35-5 - Form for writ of garnishment on judgment or decree
The writ of garnishment, when issued on a judgment or decree, may be in the following form, to wit:
"THE STATE OF MISSISSIPPI.
"To any lawful officer of ____________________ county:
"Whereas, ____________________ recovered a judgment in ____________________ court of ____________________ county, on the ____________________ day ____________________, A. D. ____________________, for the sum of ____________________ dollars and costs, against ____________________, and the judgment has not been satisfied, and said ____________________ having made the proper suggestion for a writ of garnishment against ____________________ :
"We therefore command you to summon said ____________________ to appear in said court, at ____________________, on the ____________________ day of ____________________, A. D. ____________________, then and there to answer, on oath in writing, whether (here copy in full every particular that a garnishee is required to answer). And have you then and there this writ, with your proceedings indorsed thereon.
"Witness my signature (and if by a clerk, add an official seal), this ____________________ day of ____________________, A. D. ____________________ ."

The writ must be signed by the officer who issues it, and his official character should be written after his name.
