Act 218 of 1956 500.3509 Certificate of authority; application; form; limitation; change of service area. Sec. 3509. (1) An application to the commissioner for a certificate of authority shall be on a form prescribed and provided by the commissioner. (2) A certificate of authority issued under this chapter is limited to the service area described in the application upon which the certificate of authority was issued. (3) A health maintenance organization seeking to change the approved service area shall submit an application to change service area to the commissioner and shall not change the service area until approval is received. The commissioner shall specify the information required to be in the application under this subsection.
