§ 4-504. Amendment or revocation.

(a) In general. -- Subject to § 4-506 of this subtitle, a donor or any other person authorized to make an anatomical gift under § 4-503 of this subtitle may amend or revoke an anatomical gift by:

(1) A record signed by:

(i) The donor;

(ii) The other person; or

(iii) Subject to subsection (b) of this section, if the donor or other person is physically unable to sign, another individual acting at the direction of the donor or the other person; or

(2) A later-executed document of gift that expressly or by inconsistency amends or revokes the previous anatomical gift or portion of the anatomical gift.

(b) Requirements for record. -- A record signed in accordance with subsection (a)(1)(iii) of this section shall:

(1) Be witnessed by at least two adults, at least one of whom is a disinterested witness, who have signed at the request of the donor or the other person; and

(2) State that the record has been signed and witnessed as provided in item (1) of this subsection.

(c) Destruction or cancellation of document with intent to revoke. -- Subject to § 4-506 of this subtitle, a donor or any other person authorized to make an anatomical gift under § 4-503 of this subtitle may revoke the anatomical gift by the destruction or cancellation of the document of gift, or the portion of the document of gift used to make the gift, with the intent to revoke the gift.

(d) Communications by terminally ill or injured donor. -- During a terminal illness of a donor, or while a donor is injured, the donor may amend or revoke an anatomical gift that was not made by will by any form of communication addressed to at least two adults, at least one of whom is a disinterested witness.

(e) Amendment or revocation of will. -- A donor who makes an anatomical gift by will may amend or revoke the gift in the manner provided for amendment or revocation of wills or as provided in subsection (a) of this section.

(f) Revocation of donor designation on driver's license or identification card; authorization of statement or symbol on donor registry. --

(1) An anatomical gift made by a donor designation on a driver's license or an identification card may be revoked by giving written notice to the Motor Vehicle Administration in accordance with § 12-303 of the Transportation Article.

(2) A donor may make a gift by authorizing that a statement or symbol indicating that the donor has made a gift be included on a donor registry.
