15.804 Standardized forms for entry of state case registry information. All forms, child support orders, wage withholding orders, or orders amending an existing child support order, entered in any case in Circuit Court, District Court, or family court that require entry into the state case registry under KRS 15.802(3) shall be entered on forms adopted by the Administrative Office of the Courts after consultation with the Office of the Attorney General. If the provisions of a child support order are contained in an order that is narrative in nature, the adopted forms shall be used in addition to the narrative order. Formerly codified as KRS 205.713. Legislative Research Commission Note (7/1/2025). Under the authority of KRS 7.136(1), a reference to KRS 15.802"(4)" in this statute has been changed to KRS 15.802"(3)" in codification following the enactment of 2023 Ky. Acts ch. 124, sec. 9, which repealed, reenacted, renumbered, and amended KRS 205.712 as a section of KRS Chapter 15 [KRS 15.802], deleted subsection (1) in its entirety, and resulted in the renumbering of subsequent subsections.
