61.526 Employee and employer may file statement of facts at retirement office -- Procedures for verification. (1) (2) (3) (4) Upon becoming a member of the Kentucky Employees Retirement System, the County Employees Retirement System, or the State Police Retirement System, the employee and employer may file in the retirement office, in the form and detail as the authority may prescribe, a statement of facts pertaining to the employee and other information the authority may require, including but not limited to a record of military service and previous employment with the employer. If the records of the employer employing the member during the time the service was rendered do not substantiate the statement of facts filed by the member, the member shall be notified of any discrepancy. The member shall be advised that he has the responsibility of supplying verification of any unsubstantiated service. At the request of the member, or the beneficiary if the member is deceased, the Authority's executive director shall arrange a time and place or process to receive additional information in regard to the unsubstantiated service. After filing the request, the member or the beneficiary if the member is deceased, shall have a reasonable time but no more than six (6) months to present the additional information to substantiate the unsubstantiated service. The system may allow a member to retire or obtain a refund without the member submitting a statement of facts pertaining to the member as described by this section.
