286.9-060 Materials to accompany application -- Investigation fee.

286.9-060 Materials to accompany application -- Investigation fee. Each application for a license shall be accompanied by: (1) An investigation fee of five hundred dollars ($500) for Kentucky residents and five hundred dollars ($500) for nonresidents of Kentucky for each location which shall not be subject to refund but which, if the license is granted, shall constitute the license fee for the first license year or part thereof; (2) Audited financial statements prescribed by the commissioner; and (3) Evidence that the applicant has complied or will comply with all workers', and unemployment compensation laws of Kentucky.
