212.345 Consent document -- Endorsement. Before any person in the Commonwealth may have a nontherapeutic sterilization performed upon himself, he shall give written informed consent to the performance of the sterilization. To be valid such consent document shall be endorsed by the following persons in the following instances: (1) By the consenting individual in all cases; or (2) By the adult parent or guardian of the consenting individual if such be a minor child and unmarried. Effective: July 1, 1982 History: Amended 1982 Ky. Acts ch. 141, sec. 73, effective July 1, 1982. -- Created 1974 Ky. Acts ch. 352, sec. 4. Note: 1980 Ky. Acts ch. 396, sec. 77 would have amended this section effective July 1, 1982. However, 1980 Ky. Acts ch. 396 was repealed by 1982 Ky. Acts ch. 141, sec. 146, also effective July 1, 1982.
