645.050 Contents of certification petition.

Download as PDF 645.050 Contents of certification petition. The petition for certification and all subsequent court documents shall be entitled "In the interest of ..., a child." The petition for certification shall state: (1) The petitioner's relationship to the child; (2) The child's name, age, address and present location; (3) The name and address of the child's parents or legal guardian or other person exercising custodial control or supervision; (4) The name and address of the person having custody of the child; (5) The name and address of the child's spouse, if any; (6) The petitioner's belief, including its factual basis, that the child is mentally ill and is dangerous to himself or others; and (7) The petitioner's belief, including its factual basis, that the child can benefit from treatment available only at a hospital and that no less restrictive alternative is available which will be effective in treating the child.
