(1) (2) Materials identifying the human remains placed in the custody of a crematory authority shall contain the following information: (a) Name of decedent; (b) Date and time of death; (c) Place of death; (d) Name and relationship of authorizing agent; (e) Name of person engaging crematory services; and (f) Signature of the person making the identification. Appropriate identification shall be placed upon the exterior of the cremation container. Effective:July 15, 1994 History: Created 1994 Ky. Acts ch. 140, sec. 3, effective July 15, 1994.
