(1) (2) Every applicant for a certificate or permit or for amendment or transfer or sale or change in route or abandonment of a certificate or permit shall apply for same to the department in such form and the application shall contain such matters as the department may prescribe. For the filing of such application the department shall receive a fee of twenty-five dollars ($25). Effective:July 15, 1996 History: Amended 1996 Ky.
