439.370 Commonwealth's attorney to cause to be transmitted to the institution a concise statement of facts adduced at trial or hearing. It shall be the duty of the Commonwealth's attorney after the imposition of prison sentence, unless the defendant is granted probation, to cause to be forthwith transmitted to the institution with the commitment papers, a concise statement of the facts adduced at the trial or at the hearing of a plea of guilty, together with such notes of testimony as the board may request.
