1 BUSINESS CORPORATIONS, §490.121 490.121 Forms. 1. a. The secretary of state may prescribe and furnish on request forms including but not limited to the following: (1) A foreign corporation’s application for a certificate of authority to transact business in this state. (2) A foreign corporation’s application for a certificate of withdrawal. (3) The biennial report. b. If the secretary of state so requires, use of these listed forms prescribed by the secretary of state is mandatory. 2.
