     Sec. 3. (a) A petition seeking participation of a parent, guardian, or custodian must be entitled "In the Matter of the Participation of _________ the Parent, Guardian, or Custodian of _________". The petition must allege the following:

(1) That the respondent is the child's parent, guardian, or custodian.

(2) That the child has been adjudicated a child in need of services.

     (b) A petition seeking participation of a parent, guardian, or custodian may include a recommendation that the parent, guardian, or custodian should do one (1) or more of the following:

(1) Obtain assistance in fulfilling obligations as a parent, guardian, or custodian.

(2) Provide specified care, treatment, or supervision for the child.

(3) Work with a person providing care, treatment, or rehabilitation for the child.

(4) Refrain from direct or indirect contact with the child.

(5) Participate in a mental health or addiction treatment program.
