Section 54-1735 - PATIENT MEDICATION RECORDS.

54-1735. patient medication records. In order to effectively counsel patients, the pharmacist shall make a reasonable effort to obtain, record and maintain significant patient information including, but not limited to:
(1) Name, address, telephone number;
(2) Date of birth (age), gender;
(3) Medical history:
(a) Disease state(s);
(b) Allergies/drug reactions; and
(c) Current list of medications and devices;
(4) Pharmacist comments.
