                                  CHAPTER 9
                                   DENTISTS
    54-932.  LOST OR DESTROYED CERTIFICATES OR LICENSES. If the certificate of
qualification or the license of a dentist or dental hygienist be lost or
destroyed, and such fact appear by affidavit of such dentist or dental
hygienist filed with the board together with a fee of ten dollars ($10.00),
the board shall issue a duplicate.
