§ 131. Designation by Insurer of Office for Service of Notices

The most recent address for servicing agents/claims offices submitted by an insurer, self-insured employer, or group self-insurer, on a Form WC-121, Form WC-131, Form WC-131a, self-insurer's member information annual update shall be used as the address of record for service of forms, notices, orders, and awards.
