497.146   Licensing; address of record; changes; licensee responsibility.

1497.146  Licensing; address of record; changes; licensee responsibility.--Each licensee under this chapter is responsible for notifying the department in writing of the licensee's current business and residence mailing address and the street address of the licensee's primary place of practice and shall notify the department in writing within 30 days after any change in such information, in accordance with procedures and forms prescribed by rule. Notwithstanding any other provision of law, service by regular mail to a licensee's last known address of record with the department constitutes adequate and sufficient notice to the licensee for any official communication to the licensee by the board or the department, except when other service is expressly required by this chapter. Rules may be adopted establishing forms and procedures for licensees to provide the notice required by this section.
