455.698  Reports of professional liability actions; bankruptcies; Department of Health's responsibility to provide.--

(1)  The report of a claim or action for damages for personal injury which is required to be provided to the Department of Health under 1s. 455.247 or s. 627.912 is public information except for the name of the claimant or injured person, which remains confidential as provided in ss. 2455.247(2)(d) and 627.912(2)(e). The Department of Health shall, upon request, make such report available to any person.

(2)  Any information in the possession of the Department of Health which relates to a bankruptcy proceeding by a practitioner of medicine licensed under chapter 458, a practitioner of osteopathic medicine licensed under chapter 459, a podiatrist licensed under chapter 461, or a dentist licensed under chapter 466 is public information. The Department of Health shall, upon request, make such information available to any person.
