§ 24-21-516. Short form certificates

  • (1) The following short form certificates of notarial acts are sufficient for the purposes indicated, if completed with the information required by section 24-21-515 (1) and (2):

    • (a) For an acknowledgment in an individual capacity:

    • (b) For an acknowledgment in a representative capacity:
