2005 California Labor Code Sections 1701.8 Article 3. Written Disclosure

LABOR CODE
SECTION 1701.8 

1701.8.  Prior to requesting any advance fee, an advance-fee talent
service shall provide an artist with written disclosure of all of the
following:
   (a) The name, address, and telephone number of the advance-fee
talent service, and evidence of compliance with any applicable
bonding requirements, including the bond number, if any.
   (b) A copy of the advance-fee talent service fee schedule and
payment terms.
