Clerk shall complete a Form EDKy 525 w name, inmate number and case number. Clerk shall serve a copy of this Order and form upon the Jailer/Warden in which Jones is confined and upon Office of General Counsel for Dept of Corrections in Frankfort, KY. 4) After initial partial filing fee is paid, each month send a payment.
