Notes of Advisory Committee on Rules—1991This form previously was numbered Official Form No. 34. A notation of the chapter of the Bankruptcy Code under which the case is proceeding has been added to the form. Rule 7010 refers to this form as providing the caption of a pleading in an adversary proceeding.
Notes of Advisory Committee on Rules—1995 AmendmentThe form is amended to conform to the amendments made to section 342 of the Code by the Bankruptcy Reform Act of 1994.
Committee Notes on Rules—2003 AmendmentThe form is abrogated. An amendment to Official Form 16A directs that only the last four digits of the debtor's social security number should appear in a caption. Section 342(c) of the Bankruptcy Code continues to require the debtor to provide a creditor with the debtor's name, address, and taxpayer identification number on any notice the debtor is required to give to the creditor. An individual debtor can fulfill this requirement by including the debtor's social security account number on only the creditor's copy of any notice or summons the debtor may serve on the creditor.
