     35-2-1.   Application forms and verification--Information and eligibility shown. Applications for licenses provided for by this title shall be made on forms prescribed by the secretary of revenue, and shall be verified by the oath of the applicant, if an individual, and, if a corporation, by an officer of such corporation. Such application must contain such information as the secretary requires, and must show that the applicant is eligible for the license for which application is made.
