§ 4734.55. Duplicate license; list of licensees; certification of licensure.
 

The state chiropractic board shall provide a duplicate license to a license holder on payment of a fee of forty-five dollars. 
 

Upon written request and the payment of a fee of ninety-five dollars, the board shall provide to any person a list of persons holding licenses to practice chiropractic, as indicated in its register maintained pursuant to section 4734.04 of the Revised Code. 
 

Upon written request from the licensee and the payment of a fee of twenty dollars, the board shall issue certification of licensure information to the person identified by the licensee.
