2006 New York Code - Application For License.



 
    §  26-132 Application for license. All applications for licenses shall
  be submitted  on  forms  furnished  by  the  department,  and  shall  be
  accompanied   by   the  required  fee,  as  hereinafter  provided.  Each
  application for a license shall set forth the  name,  residence  address
  and  business  address  of  the  applicant,  and  such  information  and
  supporting data concerning his or her qualifications for the license  as
  the commissioner may require.
