    §  26-108  Reports  from  different  institutions and agencies. a. All
  dispensaries and hospitals in the city shall make weekly  statements  to
  the  department as to the cases of sickness received in such hospital or
  treated in such dispensary from each building. Such statement shall show
  the location of such building by street and number, the  nature  of  the
  sickness treated, whether the patient was an adult or child and the date
  of the treatment.
    b.  The  police  department  shall  furnish to the department a weekly
  statement of the number of arrests of persons living in such  buildings.
  Such  statement  shall show the location of such building, by street and
  number, the offense with which the person is charged, the age  and  name
  of  the  offender,  and  such  other  information  as the department may
  require. The commissioner shall prescribe and furnish  blank  forms  for
  making such statements.
    c.  Such  department may require reports and information of such facts
  relative to the condition of persons residing in such buildings  as  the
  commissioner  may  deem  useful  in  carrying  out  the  duties  of  the
  department. Such reports also may be  required  from  all  dispensaries,
  hospitals,  charitable or benevolent societies, infirmaries, prisons and
  schools, and from the managers, principals  and  officers  thereof.  The
  managers,  principals  and officers of such institutions shall give such
  information promptly  and  shall  make  such  reports,  verbally  or  in
  writing, as the commissioner may require.
