Act 345 of 1990 54.273 Application for annual grant; form. Sec. 13. An application for an annual grant authorized pursuant to this act shall be made on a form prescribed and furnished by the commission. The commission may require the applicant to provide any information reasonably necessary to allow the commission to make a determination required by this act.
