Maryland Health Occupations Section 17-303

      An applicant for a certificate shall:

            (1)      Submit an application to the Board on the form that the Board requires;

            (2)      Pay to the Board the application fee set by the Board; and

            (3)      Furnish to the Board the professional disclosure statement required under § 17-312 of this subtitle.
