§2008.  Annual report

A.  On or before the first day of March, every health maintenance organization shall annually file a report with the commissioner covering the preceding calendar year and shall file a copy of the report with the secretary of the Department of Health and Hospitals.  The report shall be verified by at least two principal officers or duly authorized representatives of the health maintenance organization.  The report shall be on forms prescribed by the National Association of Insurance Commissioners.  The report shall be accompanied by the filing fee and any penalty for unauthorized late filing set forth in R.S. 22:2024.

B.  The commissioner for good cause may grant an extension of up to thirty days for the filing of the annual report.

C.  In addition to Subsection A of this Section, the following reports shall also be filed with the commissioner:

(1)  Quarterly statements as required by R.S. 22:1451.

(2)  Management and discussion reports as required by R.S. 22:1451 and the National Association of Insurance Commissioners Annual Statement Instructions Handbook.

(3)  A CPA audited report as required by R.S. 22:1324 and Regulation 50 issued by the Department of Insurance.

(4)  Holding Company Act filings as required under Part XXI-A of Chapter 1 of Title 22 of the Louisiana Revised Statutes of 1950.
