§1210.61. Registration forms; submissions
A. Each PEO operating in the state prior to or on September 1, 2001, shall submit a registration form to the Department of Insurance by October 30, 2001, and to the Department of Labor by January 31, 2002, or within thirty days after the registration form is made available by the respective departments, whichever is later.
B. Any PEO which commences operations after September 1, 2001, shall submit a completed registration form to the Department of Insurance and the Department of Labor in advance of commencing operation in this state or within thirty days after such form is made available by the respective departments, whichever is later.
