Any individual who forges or fraudulently alters a form completed by the prospective registrant, who willfully destroys a completed registration form or who willfully fails to return a completed form to the county clerk for official registration by the county clerk shall be subject to the penalties described for Class D felonies. Effective:June 19, 1976 History: Created 1976 Ky. Acts ch. 30, sec. 2, effective June 19, 1976.
