311.679 Reporting and recordkeeping requirements.

(1) An acupuncturist shall comply with all applicable state and municipal reporting requirements imposed on health-care professionals regarding public health. (2) An acupuncturist shall maintain a record for each patient treated, in a manner consistent with the acceptable and prevailing standards of practice within the <br>Commonwealth. At a minimum, the record for each patient shall include: <br>(a) A signed copy of the information disclosed by the acupuncturist to the patient under KRS 311.678; (b) Evidence that the acupuncturist has conducted or overseen an interview concerning the patient's medical history and current physical condition; (c) Evidence of the acupuncturist having conducted a traditional acupuncture examination; (d) A record of the treatment, including the acupuncture points treated; and <br>(e) The evaluation and instructions given.
