All health benefit plans issued or renewed on or after July 15, 1995, which provide <br>coverage for a family member of the insured shall provide that the health plan benefits <br>applicable for children shall be payable with respect to legally-adopted children of the <br>insured or any child for which the insured is a court-appointed guardian from and after <br>the date of the filing of the petition for adoption or the filing of the application for <br>appointment of guardian. Effective: July 15, 1994 <br>History: Created 1994 Ky. Acts ch. 512, sec. 57, effective July 15, 1994.
