213.078 Questions concerning diabetes on certificate of death form. (1) Any certificate of death form developed or distributed by the Cabinet for Health and Family Services shall contain the following questions: (a)  "Did the deceased have diabetes?"; and (b)  "Was diabetes an immediate, underlying, or contributing cause of or condition leading to death?". (2) If the person completing the certificate of death fails to answer the questions identified in subsection (1) of this section, the state registrar shall call attention to the defects in the certificate and require the person responsible for the entry to complete or correct it.
